Location: Quito & CuencaThe U.S. Mission in Ecuador is pleased to announce that representatives from the Regional Federal Benefits Unit will visit Quito and Cuenca to offer services for U.S. Social Security and other federal benefits.
Quito: Location: Centro Ecuatoriano Norteamericano (CEN) Av. Amazonas y La Prensa (Antiguo Aeropuerto Quito / Old UIO Airport Location in North Quito Downtown) Dates: Monday, May 12, 2025, from 8:00 AM to 4:00 PM - Tuesday, May 13, 2025, from 8:00 AM to 4:00 PM
Cuenca: Location: Abraham Lincoln Cultural Center, Calle Borrero 5-18 y Honorato Vásquez, CuencaDates: Wednesday, May 14, 2025, from 8:00 AM to 4:00 PM
Thursday, May 15, 2025, from 8:00 AM to 4:00 PM
You don’t need to schedule an appointment for your visit. We will provide a limited number of tickets per day for services with a representative on a first come, first served basis.
If you only wish to submit your 2024 Foreign Enforcement Questionnaire SSA 7162/7161 form, you may drop the form during service hours at the above locations. For this specific service, you will not need a ticket since you will not meet with a representative.
You must bring original documents as well as a photocopy of each document required for your application.
How to prepare for your visit: Foreign Enforcement Questionnaire (FEQ): FBU will accept the 2024 “proof of life” forms. If your Social Security Number ends in 00 through 49 and you have a mailing address registered in Ecuador, you are required to submit this form. If you didn’t return this form by January 2025, your benefits were or will be suspended.
Application for Social Security Benefits: For each applicant, bring an original and a copy of the birth certificate and U.S. passport. If applying for auxiliary benefits or survivor’s benefits as the spouse of a deceased beneficiary, bring the original and copy of the marriage and death certificates.
Application for Social Security Number Card: For each applicant, bring an original and a copy of the local birth certificate, U.S. passport, and completed form SS-5FS.
